Incomplete Post-Dialysis Assessment for Two Residents
Summary
The facility failed to complete post-dialysis assessments for two residents who returned from hemodialysis, including assessment of the dialysis access site and vital signs. Resident 1 was admitted with diagnoses including type 2 DM, CKD, and dependence on renal dialysis, and had orders to monitor the AV fistula/graft site for signs of infection, edema, and bleeding upon return from dialysis, along with monitoring vital signs. Resident 3 was admitted with diagnoses including ESRD, dependence on renal dialysis, and fitting and adjustment of an extracorporeal dialysis catheter, with care plan and orders directing staff to monitor the hemodialysis site for complications such as bleeding, swelling, pain, drainage, odor, hardness, or redness. For Resident 1, the Hemodialysis Communication Record showed post-dialysis assessments on two occasions with no vital signs documented and no assessment of the access site. For Resident 3, the Hemodialysis Communication Record also showed post-dialysis assessments on two occasions with no vital signs documented and no assessment of the access site. The residents' MDS assessments indicated they were able to usually understand and usually be understood. During a concurrent interview and record review, the DSD stated that dialysis residents have a communication binder and that upon return from dialysis the nurse must check vital signs and the dialysis sites and document on the binder. The DSD reviewed the records for both residents and stated the post assessment was not done, could mean the residents were not stable and the facility would not know, and could delay care. The facility policy for hemodialysis required ongoing assessment before and after treatment and specifically required monitoring and documentation of the vascular access site, vital signs, and post-dialysis complications.
