During an observation on 9/29/2025 at 8:24 AM, staffing information dated 9/26/2025 was posted in the hallway near the nursing station. During a concurrent observation and interview at 9:35 AM, the Registered Nurse Supervisor verified that the posted staffing information was still dated 9/26/2025 and stated that the staffing information for the weekend and for 9/29/2025 had not been posted. During an interview on 10/1/2025, the Director of Staff Development stated she did not know why the staffing information printed for 9/27/2025, 9/28/2025, and 9/29/2025 was not posted in the hallway and said it had been left in the binder instead. She stated that the posting should include the census and the total number of RN, LVN, and CNA staff working each shift and should be easily seen and read by residents, visitors, and staff. The facility's policy titled Posting Direct Care Daily Staffing Numbers, revised in August 2022, required daily posting of nurse staffing data, including the number of nursing personnel responsible for providing direct care to residents.

On 6/3/2026, staffing information dated 6/4/2026 was posted at the 100 Hall nursing station near the off-hours facility entrance. During interview, the DON confirmed the staffing information posted was dated 6/4/2026 and stated the scheduler would prepare the staffing information sheet before leaving and corrections would be made the next day. The DON also stated the staffing sheet should not be posted in the front and should be placed behind the current day until the new shift starts. Administration stated the staff coordinator should not post the staffing information the day before and should post the current staffing information for the day. The Staffing Coordinator stated she had been putting the staffing information out before leaving at the end of the day for the following day and did not notice the census was incorrect.

During interview, NA-A verified the posting was dated 5/26/26 and stated it was important for families and visitors to see how many staff were working. SC-B stated she changed the nurse staffing hours on Thursdays for the coming week but did not update the posted sheets, and instead made the changes in the computer after they occurred when she returned on Mondays. A review of the next week's schedules showed the census as 24, and one schedule dated 5/30/26 had zeros for all staff listed with a census of 24. The DON verified the nurse staff posting should be posted daily so anyone could see how many residents were in the facility and how many staff were working, and stated it was important so families could have confidence there was enough staff to take care of their family members.
