Incomplete 15-Minute Safety Check Documentation for Resident on 1:1 Supervision
Summary
The facility failed to ensure that Resident 2’s medical record was complete and accurately documented when the resident’s every 15-minute safety checks were not consistently recorded by staff on 8/18/25, 8/20/25, and 8/25/25. Resident 2 was admitted with diagnoses including paranoid schizophrenia, insomnia, major depressive disorder, and anxiety disorder. The care plan identified a history of psychosocial distress related to Vietnam war service, homelessness, schizophrenia, stroke, and physical aggression triggered by paranoia, including resident-to-resident altercations documented on multiple dates earlier in the year. The record showed that Resident 2 had a care plan goal to reduce or decrease episodes of PTSD triggered by altercations and an intervention for 1:1 staff support as indicated. A later care plan entry stated the resident had possible resident-to-resident altercation risk, with a goal of having no episodes for 30 days and continued 1:1 supervision. During interviews, CNA 1 stated staff assigned to 1:1 supervision were responsible for documenting the resident’s location and status every 15 minutes and should not leave any time frame blank. CNA 2 acknowledged that the every 15-minute safety checks were not consistently documented on the identified dates and stated the resident was on 1:1 supervision 24/7 to prevent further altercations. Licensed Nurse 1 stated the resident had been on 1:1 supervision since June 2025 due to multiple altercations with other residents and that staff were accountable for staying with the resident and documenting every 15 minutes. CNA 4 stated staff should document resident status every 15 minutes and that incomplete documentation might state the resident was left unattended. During review with the Administrator, the facility confirmed staff did not follow the provided documentation guidelines, and the Q 15 MINUTE SAFETY CHECKS forms for the identified dates were not consistently documented. The facility’s ONE-ON-ONE CARE ACKNOWLEDGEMENT FORM stated that proper supervision and adherence to the guidelines were critical in maintaining resident safety and preventing resident-to-resident altercations.
