Infection Control Failures With Glucometer Handling, Hand Hygiene, and EBP Signage

Summary

The facility failed to maintain an infection prevention and control program in several observed practices involving blood glucose monitoring, hand hygiene, and enhanced barrier precautions. During observations, two LVNs did not consistently follow infection control measures while checking fingerstick blood glucose on residents. One LVN placed a glucometer directly on a resident’s overbed table, then later placed the soiled glucometer on top of the medication cart without a barrier. The same LVN also placed the glucometer and supplies directly on a resident’s bed. Another LVN placed a drape on the medication cart and a paper towel on the overbed table, but after checking the resident’s blood glucose, placed the soiled glucometer directly on top of the medication cart without a barrier. The DON stated the glucometer should be disinfected between residents and placed on a barrier such as a paper towel or tissue. Hand hygiene was also not consistently performed. A CNA was observed entering a resident’s room without performing hand hygiene, assisting the resident with the bed and touching the resident’s environment, and then leaving the room without performing hand hygiene. The CNA stated hand hygiene should be done every time staff enter or exit a resident’s room and before and after contact with the resident or belongings. The facility’s hand hygiene policy stated hand hygiene is the primary means to prevent the spread of infections and should be performed before and after direct contact with residents and after contact with objects in the resident’s immediate vicinity. Two rooms with enhanced barrier precautions did not have signage posted to notify staff of the required precautions before entering. PPE carts were observed in front of the rooms without signs indicating the reason for the PPE cart or the PPE required. Staff stated the signs were missing and should have been posted above the PPE carts so staff would know what precautions to take. The facility’s EBP policy stated signs are posted in the door or wall outside the resident room indicating the type of precautions and PPE required, and the CDC reference stated signs are intended to signal the specific actions staff should take when caring for the resident.
