Infection Control Lapses During Resident Care

Summary

The facility failed to maintain its infection prevention and control program during multiple observed resident care activities involving five residents. During incontinent care for a resident with a dialysis catheter and Enhanced Barrier Precautions, a CNA did not wear a gown, did not perform hand hygiene before entering the room, and did not change soiled gloves or wash hands between tasks while cleaning the resident and changing the brief. The CNA also left the room and entered another resident’s room without washing hands or using hand sanitizer. The CNA stated she was in a hurry because the resident had to be ready for dialysis transport and acknowledged she forgot to use the required gown and hand hygiene measures. During tube feeding care for a resident with a G-tube and severe cognitive impairment, an LVN placed supplies on the overbed table without cleaning it first, even though the table had a dried dark sticky substance on it. The LVN completed replacement of the tubing, spilled formula on the table, and left the room without cleaning the overbed tabletop afterward. The LVN stated she knew the table should have been cleaned before placing clean supplies on it and that if the surface was not clean it could spread germs to the new supplies. An LVN also failed to disinfect shared equipment between residents and during care. The LVN did not sanitize the blood pressure cuff before or after checking blood pressure on one resident with severe cognitive impairment and multiple diagnoses, then later failed to sanitize the same cuff before or after using it on another resident. The LVN likewise failed to sanitize the glucometer before or after blood sugar checks on two residents with diabetes. During tracheostomy care for a resident with severe cognitive impairment and acute respiratory failure, the LVN entered the room without washing hands or using hand sanitizer, changed gloves without hand hygiene, and later removed gloves and again placed on another pair without washing hands or using hand sanitizer. The LVN stated she had gotten in a hurry and had not followed the infection control protocol, and acknowledged that the equipment should be cleaned with sanitizing wipes before and after each use.
