Summary

The facility failed to store, prepare, distribute, and serve food in accordance with professional food service standards in the kitchen. During observation of the walk-in food storage room, surveyors found a box of graham cracker crumbs in an unsealed zip-top bag, 12 slices of white bread in an unsealed and unlabeled bag, an opened ketchup bottle that was only half full and required refrigeration after opening, and a bag of cake mix icing in an unsealed manufacturer’s bag and another 5 lbs. bag in an unsealed zip-top bag with no date. In the same storage area, two cans of Chunk Light skip jack tuna had dents on the seals and no best buy or use-by date, a can of pinto beans had a dent on the bottom seal, and a can of sliced beets had rust around the top and bottom of the seal with no best buy or expiration date. In the walk-in refrigerator, three large produce boxes were observed without dates. One box contained cucumbers, another cantaloupes, and the third zucchinis. The produce in all three boxes was collapsing in areas, slimy to the touch, wrinkled, discolored, and had visible fuzzy mold growth. The boxes were soggy, stained, and wet from decomposing juices, with dark spots spreading across the cardboard. In the walk-in freezer, surveyors observed a large bag of meat patties with no label and no date, and a box of chicken thighs stored in an opened plastic bag exposed to air, with the bag dated July 22nd. During interviews, the DM stated that dented cans go in her office and that all staff are responsible for removing dented cans if they notice a dent. She also stated that if there is no date on a can, there is a code that can be looked up to determine the date, and that residents could become ill if expired or improperly stored food is consumed. The DM further stated that kitchen staff who remove food items from the freezer, refrigerator, or dry storage are responsible for putting the food item back labeled with the open date and properly sealed. Facility policies for refrigerator/freezer storage and dry storage required items to be dated, labeled, tightly sealed, and discarded according to the stated timeframes, and the FDA Food Code cited that rusted, pitted, or dented cans may present a serious potential hazard.
