Nurse Staffing Information Not Posted Daily
Summary
The facility failed to ensure Nurse Staffing Information was posted daily for one of one building. During an observation on 08/12/25 at 04:35 AM, there was no Nursing Staffing Information posted in an area visible to all residents and visitors. The report states the facility did not post and maintain the required staffing information on August 12, 2025. During an interview on 08/12/25 at 11:42 AM, the Staffing Coordinator stated she places the staffing sheets every morning when she arrives and adjusts them if there is a call out. She stated the sheets are placed in a holder on the wall near the DON's office, that she also does the weekend sheets, and that they are accessible for the weekend supervisor or charge nurse to place and/or update if needed. She stated she had placed it that morning but did not know who removed it, and she brought the actual sheet to the surveyor for proof. In an interview on 8/14/25 at 1:09 PM, the Administrator stated they always have the information posted and that the staffing coordinator updates it. Record review of the facility policy, Posting Direct Care Daily Staffing Numbers, stated the facility will post on a daily basis for each shift the number of nursing personnel responsible for providing direct care, and within two hours of the beginning of each shift the number of licensed nurses and unlicensed nursing personnel will be posted in a prominent location accessible to residents and visitors.
