Summary

Facility staff failed to ensure that CNA E demonstrated competency in the technique needed to provide incontinent care for Resident #66. Resident #66 was admitted on 04/01/2024 and readmitted on 02/19/2025 with diagnoses including alcoholic cirrhosis of the liver, dysphagia, sepsis, dementia, hyperlipidemia, hypertension, and major depressive disorder. The resident's quarterly MDS dated 07/02/2025 showed a BIMS score of 03, indicating severe cognitive impairment, and the resident required total assistance with ADLs and was always incontinent of bowel and bladder. The care plan identified the resident as incontinent of bladder and bowel and at risk for skin breakdown/irritation, with an intervention for incontinent care as needed and application of moisture barrier after each episode. During observation, CNA E provided incontinent care and wiped the resident's buttocks in a back-to-front motion. In interview, CNA E confirmed she had used the wrong motion and stated it could cause a risk for infection; she also confirmed receiving training on incontinent care from the facility. The DON confirmed that the correct motion for perineal care was front to back to prevent fecal matter from contacting the urethra and possibly causing an infection. The report also noted that CNA E had passed annual competency for perineal care/incontinent care on 08/01/2024, and the facility policy required care to the buttocks and anal area to be performed from front to back without contaminating the perineal area.
