Summary

The facility failed to conduct the required Preadmission Screening and Resident Review (PASARR) for a resident with multiple mental health diagnoses, including schizophrenia, bipolar disorder, and major depression. The resident was admitted with these diagnoses and was receiving psychotropic medication.
