For another resident, who had diagnoses of delusional disorder and schizophrenia, the facility did not complete a new Level I PASRR screening after the resident was diagnosed with SMI. The resident's records showed no evidence of a Level I screening following the updated diagnoses, and staff confirmed that the required screening was not performed. The facility's own policy requires timely submission and follow-up of PASRR documentation, but this was not adhered to in these cases.
