Summary

The facility failed to maintain a clean, sanitary, and homelike environment for several residents, as evidenced by the presence of a hole in the wall underneath the bathroom sink shared between two rooms. This hole, located adjacent to plumbing pipes, was left open after maintenance staff accessed the plumbing to fix a clogged toilet. The Maintenance Supervisor confirmed that the hole remained open and acknowledged that flies could enter the facility through this and other gaps, such as opened windows and sliding doors.
