Summary

The facility failed to ensure that the call light for one resident was accessible and within reach, as required by the resident's care plan and facility policy. The resident had significant medical conditions, including lumbar spondylosis, neuropathy, and respiratory failure, and was dependent on staff for activities such as eating, toileting, personal hygiene, and dressing. The care plan specifically identified the resident as being at risk for falls and required that the call light be kept within reach to allow the resident to request assistance as needed. During an observation, the call light was found on the floor behind the resident's bed, out of reach. Staff interviews confirmed that the call light should be accessible to the resident at all times, and the facility's policy required the call light to be within reach when the resident is in bed.
