Summary
The facility failed to ensure that the Physician Orders for Scope of Treatment (POST) form for one resident was completed according to the requirements specified by the West Virginia Center for End-of-Life Care and the West Virginia Health Care Decisions Act. Specifically, a review of the resident's medical record revealed that Section D of the POST form, which mandates the signature of the resident, guardian, or Medical Power of Attorney (MPOA), was not properly completed.
