Summary

The facility failed to ensure that nurse aides completed the required 12 hours of annual education, as evidenced by a review of personnel files during the annual survey. Specifically, one nurse aide was found to have completed only 9 hours and 34 minutes of education in the calendar year 2024, including just 24 minutes of dementia care education. When the Director of Nursing was asked to provide documentation of any additional education for this nurse aide, no further evidence was supplied prior to the survey exit.
