Documentation indicated that nail care was not completed as required, with several referrals to nursing staff and no evidence of trimming or cleaning. While some refusals were documented on certain dates, there were additional dates where nail care was not provided and no refusals were recorded. Staff interviews confirmed that nail care was to be performed on shower days, but the resident did not refuse care on the dates in question.
