Summary

Staff failed to provide care and services in a respectful and dignified manner for a resident who was admitted with a history of stroke and right shoulder joint replacement. The resident's comprehensive assessment indicated intact cognition and a need for one-person assistance with activities of daily living, including toileting. Despite being continent of bowel upon admission, the resident reported having to wait an extended period for help to use the restroom, resulting in incontinence and feelings of embarrassment. Review of the resident's care plan specified that the resident should be toileted on awakening and as needed, consistent with their continent status. However, nursing assistant task reports documented episodes of bowel incontinence on two occasions.
