Summary

Facility staff failed to post the most recent survey results in a location that was readily accessible to residents, family members, and legal representatives. During a Resident Council interview, residents were unaware of where to find the most recent State survey report. The Social Worker also stated she did not know the location of the survey results and had not educated residents about it during monthly resident council meetings. An observation tour revealed that a sign in the facility lobby directed individuals to a labeled bookcase drawer near the living room fireplace and card room area, but the sign was surrounded by books and not easily visible. The survey results were found in a brown cabinet next to the living room fireplace, with a sign above the cabinet indicating their location, but the visibility and accessibility of this information were inadequate. Facility leadership, including the Administrator, was unable to confirm how many residents could easily view the sign or access the survey results. No additional information or concerns were provided by staff during the final interview.
