Summary

The facility failed to post daily staffing sheets that included the actual number of hours worked by Registered Nurses (RN), Licensed Practical Nurses (LPN), and Certified Nursing Assistants (CNA) for a period of 31 consecutive days. Review of the posted daily staffing sheets from 7/12/2025 to 8/11/2025 showed that the required information was missing.
