Summary
Surveyors found that the facility failed to discard expired medications in both medication storage rooms observed. During an observation in the South Hall medication room, an opened bottle of Cranberry tablets with an expiration date of 4/2025 was found on the shelf and available for use. The LPN present confirmed that the bottle was expired and should have been discarded after its expiration date. The Director of Nursing also confirmed that the expired Cranberry tablets should not have been available for use. In a separate observation in the Main medication room, an opened bottle of Magic Mouthwash labeled with a 'Do Not Use after' date of 4/16/2025 was found in the refrigerator. The LPN stated that the medication had likely been discontinued and confirmed it should have been discarded after the specified date. The DON also confirmed that the Magic Mouthwash was expired and should have been removed according to facility policy.
