Summary
The facility failed to provide a homelike environment for one resident who was admitted with diagnoses including hemiplegia, dysphagia, and intracranial injury, and who was noted to be severely impaired in cognitive skills for daily decision making. Observations in the resident's room revealed a large piece of wallpaper missing from the wall beside the bed, a large area of missing paint near the head of the bed, and wallpaper peeling away from the baseboard by the window.
