Summary

The facility failed to develop a baseline care plan within 48 hours of admission for one resident, as required by its own policy. The resident was admitted with a medical history including cerebral infarction, hyperglycemia, aphasia, and gastrostomy status, and presented with pressure areas on several parts of the body and non-pressure areas on the back and left ear.
