The residents involved had significant medical histories, including recent fractures, post-surgical symptoms, high blood pressure, atrial fibrillation, cerebral infarction, COPD, and diabetes mellitus. Despite the facility's policy requiring a safe, clean, and comfortable environment, the observed deficiencies in room maintenance and sanitation were not addressed, resulting in a failure to provide a homelike and sanitary environment for the affected residents.
