Summary
The facility failed to provide the required 12 hours of annual in-service education within 12 months of the hire date anniversary for five nurse aides. Review of staff education records and facility-provided documents showed that each of these nurse aides received between four and nine hours of in-service education during the relevant 12-month periods, falling short of the regulatory requirement.
