Despite recommendations from a psychiatric nurse practitioner and the resident's own request for a referral to outpatient mental health therapy due to increased anxiety, there was no evidence that staff notified the physician or made the referral. The Director of Nursing confirmed that no referral had been made, and the resident reported ongoing increased anxiety and an unmet request for therapy.
