The facility failed to maintain sanitary food service conditions in the first-floor dining room by not ensuring that staff adhered to established personal appearance standards. According to facility policy, male staff with facial hair are required to wear a beard guard while working in the production kitchen, defined as the area where food is prepared or plated. During multiple meal service observations, the Nutrition Service Manager, who had a beard, was seen working in the production kitchen without a beard guard. On one occasion, he wore a surgical mask, but his facial hair was still exposed and not fully covered. The Administrator confirmed that the Nutrition Service Manager should have been wearing a beard guard while in the production kitchen.
