Summary
The facility failed to develop and implement a baseline care plan within 48 hours of admission for one of two new admissions reviewed. According to facility policy, a baseline plan of care is required to address each resident's immediate health and safety needs within forty-eight hours of admission. Review of the clinical record for a resident admitted with a diagnosis of dementia showed that while a comprehensive care plan was initiated several days after admission, there was no evidence of a baseline care plan specifically addressing dementia.
