Summary
The facility failed to maintain acceptable parameters of nutritional status for two of five residents reviewed. According to facility policy, all residents are to be weighed on admission, readmission, and at least monthly. For one resident, an admission weight and a mini nutritional assessment were completed, which identified the resident as being at risk for malnutrition, but no subsequent weights were obtained. For another resident, the only documented weight was recorded prior to admission, with no further weights obtained after admission.
