Summary
The facility failed to ensure that nurse aides received the required 12 hours of annual in-service training, as evidenced by a review of employee education records and staff interviews. Specifically, two out of three nurse aides reviewed did not meet the annual training requirement: one received only 9.5 hours and the other 11 hours of in-service training in the past year.
