Summary
The facility failed to develop and implement a comprehensive care plan that addressed all identified needs for one resident. Clinical record review showed that the resident was admitted with diagnoses including atrial fibrillation, multiple wounds, heart disease, and a skin infection, and was dependent on staff for care.
