The facility failed to provide specialized rehabilitative services, including physical, occupational, and speech therapy, as ordered for three residents who required these services. Staff acknowledged the orders were missed but could not provide a reason for the omission. A third resident with hemiplegia and aphasia had orders for PT, OT, and SLP evaluations and treatments. The therapy schedules indicated multiple missed sessions across all three disciplines, with staff citing therapist absences and lack of documentation for the missed sessions. Family members and staff confirmed the resident did not receive therapy as frequently as ordered, and the administrator acknowledged the missed sessions.
