Summary
The facility failed to ensure that a care plan was appropriately revised following a behavioral incident involving a resident. On 06/05/25, a behavior note documented that a resident with diagnoses of bipolar disorder and anxiety engaged in inappropriate physical contact with another resident in the dining room and later attempted similar behavior with a staff member. The resident was redirected and returned to their room. Although the care plan was revised the following day to note the incident, no new interventions were put in place to address the behavior.
