Summary
The facility failed to report an allegation of abuse to the Oklahoma State Department of Health (OSDH) within the required two-hour timeframe, as specified in their abuse policy. Record review showed that a resident with diagnoses including bipolar disorder and anxiety was involved in an incident where they inappropriately grabbed another resident's hand and placed it in their pants in the dining room.
