Summary

The facility failed to maintain a clean and safe environment, as evidenced by observations and staff interviews. One shower room was found to have three tiles missing from the wall, one tile missing from the floor, and visible mold in the corners of the shower stall. A Certified Nursing Assistant confirmed these findings during an early morning observation.
