A Certified Nursing Assistant confirmed these findings during an early morning observation. The Maintenance Director reported that a grab bar had been pulled off the wall weeks prior and that repairs were delayed due to an expired company credit card, which prevented the purchase of necessary materials. The Regional Maintenance Director confirmed that the facility had not yet received new credit cards. Review of the maintenance request log also documented the broken grab bar in the resident shower room. These issues had the potential to affect all 50 residents in the facility.
