Summary

The facility failed to provide necessary social services to support the mental health and psychosocial well-being of residents following traumatic incidents. Three residents with varying diagnoses, including depressive disorder, metabolic encephalopathy, and anxiety disorders, experienced incidents such as alleged abuse, unexplained bruising, and theft of personal property.
