Certified Nurse Aides confirmed that all residents on their respective halls used these central shower rooms and that many residents had complained about the flooring, which caused the shower chair wheels to jerk or get stuck. The facility administrator acknowledged the presence of missing tiles and holes in all central shower rooms, affecting all residents who use these areas for bathing.
