Observations and interviews confirmed that the resident regularly smoked unsupervised and considered himself an independent smoker. Facility staff, including the Unit Manager and MDS nurses, acknowledged the inconsistency between the care plan and the resident's actual smoking status. The MDS nurses agreed that the care plan should clearly indicate whether the resident was a safe, independent smoker or required supervision, but at the time of the survey, the care plan remained inaccurate.
