Summary

A resident with intact cognition was readmitted to the facility and underwent several Minimum Data Set (MDS) assessments, including quarterly and annual reviews. Record review revealed there was no evidence that the resident was invited to participate in care plan meetings following these assessments. The resident reported having attended care plan meetings in the past but could not recall the last time one was held, and expressed a desire to participate in future meetings to provide input about his care.
