Summary
The facility failed to update the care plan for a resident who was admitted with diagnoses including Type II diabetes, dementia, and hypertension. Physician orders dated 05/08/25 indicated that the resident required continuous oxygen at 2 liters per minute via nasal cannula, with instructions to maintain oxygen saturation above 90%.
