The resident stated that the velcro had been present since she moved into the room approximately two months prior, and she also noted the condition of the wall and the uncovered outlet. During an interview, the facility administrator confirmed the presence of the velcro, the scraped and unpainted wall, and the missing outlet cover, stating that the velcro was previously used to hang an activities calendar, but no calendar was currently present. The administrator was unaware of the wall damage and the missing outlet cover prior to the surveyor's observation.
