Summary

The facility failed to maintain an accurate and complete medical record in accordance with accepted professional standards by not updating a resident's Comprehensive Care Plan (CPP) to include a documented fall and corresponding interventions. Specifically, a resident with multiple diagnoses, including emphysema, alcoholic cirrhosis with ascites, severe protein-calorie malnutrition, hypertension, depression, and anxiety disorder, was found on the floor beside their bed. The resident, who had severely impaired cognition as indicated by a BIMS score of 0, denied pain and had vital signs and neurological checks performed following the incident. Despite a late entry in the progress notes stating that the care plan was updated, a review of the care plan revealed no documented evidence that it was revised to reflect the fall or to include interventions addressing the incident.
