Facility policy requires that a signed and dated copy of the bed-hold notice be kept in the resident's file and that transfer/discharge notices be provided in an understandable manner to the resident and their representative, with evidence that the notice was sent to the Ombudsman. Review of the records and staff interviews confirmed that these requirements were not met for the two residents involved.
