Summary

Facility staff failed to complete an admission Minimum Data Set (MDS) assessment in a timely manner for one resident. The resident was admitted with multiple diagnoses, including vascular dementia, type II diabetes mellitus, major depressive disorder, Alzheimer's disease, cerebral atherosclerosis, and a history of mini stroke.
