For Resident #31, the facility failed to implement care plan interventions for keeping skin clean and dry and providing prompt care after each incontinent episode, and also failed to provide appropriate incontinence care to maintain comfort. The facility also failed to store, label, and maintain food in a sanitary manner during a kitchen observation.
