Summary

The facility failed to provide required 1:1 feeding assistance to a resident with multiple sclerosis, dysphagia, and impaired cognition, as documented in their medical record and diet order. On two separate occasions, the resident was observed eating lunch unassisted, despite a meal ticket and diet order specifying the need for 1:1 feeding assistance.
