Summary

The facility failed to ensure that a Preadmission Screening and Resident Review (PASRR) was accurately completed prior to the admission of a resident with a diagnosis of Schizoaffective Disorder and a history of alcohol use. The resident was admitted with documented mental health diagnoses, and hospital records indicated recent changes to antipsychotic medication.
