Summary

The facility failed to complete a Significant Change in Status Minimum Data Set (MDS) assessment within the required 14-day timeframe for a resident who was admitted to hospice care. Record review showed that the resident had been in the facility since May 2023 and was admitted to hospice on 1/22/25. The Significant Change in Status MDS assessment was not completed and signed until 2/19/25, which was 29 days after hospice admission and 15 days past the required deadline.
