Summary

The facility failed to ensure that Geriatric Nursing Assistants (GNAs) received required annual performance reviews and at least 12 hours per year of in-service training, as identified through record review and staff interviews. Specifically, two GNAs did not have documentation of annual performance appraisals or the mandated in-service education for the years 2022-2025.
