Summary

Surveyors observed significant ice buildup in two cold storage areas within the facility's kitchen. In the main kitchen's walk-in freezer, ice was found on the floor and covering approximately 75% of the ceiling. Dietary staff present during the observation was unaware of the preventive maintenance schedule for the freezer room. Additionally, in the second-floor nourishment room, the refrigerator/freezer used to store residents' outside food had more than an inch of ice accumulation around the freezer. Residents' snacks were also stored in this refrigerator. These findings were based on direct observations and staff interviews during the survey period.
