Summary

The facility failed to maintain the kitchen in a clean and sanitary condition as required. During multiple days of survey, surveyors observed several deficiencies including an open bag of cookie dough in the reach-in refrigerator that was neither labeled nor dated, stained ceiling tiles, rust-covered ceiling support struts, and cobwebs containing insects hanging from the ceiling. There was also a significant accumulation of dust and debris on ceiling tiles, particularly near the dish machine, which became airborne when disturbed. Additional observations included dust strings hanging from the ceiling, large cobwebs in various areas, a dust-covered glove holder positioned over clean bowls, and a large floor mixer covered with a plastic bag that, when removed, revealed dried debris around the machine. Dust was also noted on the exterior of the ice machine. These findings were confirmed by the Food Service Director during the survey. No information about residents or their medical conditions was included in the report.
