Summary
Surveyors observed and confirmed multiple deficiencies related to the maintenance of a safe, clean, comfortable, and homelike environment across all three units of the facility. On the third floor, issues included missing corner protectors, stained ceiling tiles in several rooms, gouged and damaged walls, holes in bathroom walls, and broken baseboards with missing tiles in the bathing suite. The second floor had holes in the walls behind the coffee bar in the common area, stained ceiling tiles in bathrooms, bathroom doors with holes, slow-to-heat water in a resident room, and multiple holes in a bathroom wall. On the first floor, deficiencies included stained ceiling tiles in the sun room, a damaged counter under a sink, bathroom doors and walls with holes, and a stained ceiling tile above a toilet. These findings were based on direct observation and confirmation during a facility tour with the Administrator. No specific residents or staff were identified as being directly involved in the observed deficiencies, and no additional medical history or resident conditions were noted in relation to the findings.
